Trenton Health Team (THT) is an innovative multi-sector partnership dedicated to the health and well-being of the greater Trenton community. Nationally-recognized for results achieved over more than ten years of work, THT focuses on bringing organizations and individuals together to improve both the social determinants of health and the regional healthcare system. THT was founded as a partnership among St. Francis Medical Center, Capital Health, Henry J. Austin Federally Qualified Health Center, and the City of Trenton Department of Health and Human Services.
